Initial Consultation
Your initial appointment will consist of a complete review of your medical history, a thorough exam followed by an explanation of your diagnosis and treatment options. Occasionally, surgery can be done the same day as the consultation. However, certain surgical situations require advanced planning or scheduling and a consultation may be necessary prior to surgery. Some examples include; complex medical histories, intricate surgery, request for sedation, biopsies, or multiple procedures. This consultation allows the surgeon to review the treatment, discuss the risks, benefits and alternative treatments and appropriately schedule the surgery on another day.
Please assist us by providing the following information at the time of your consultation:
1. Your medical history, medications and allergies can be filled out using the secure online form prior to your scheduled appointment.
2. Please bring a form of identification with you picture on it. A copy of this identification will be made at your appointment for patient identification and insurance verification.
3. Your surgical referral slip and any x-rays, if applicable.
4. If you have any medical or dental insurance, bring your insurance cards.
Providing this information will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at all visits, both consultation and surgical. A parent must be present at the consultation to review the risks, benefits and alternatives to treatment. A parent or legal guardian must be present at the time of surgery to sign the informed consent form.
A pre-operative consultation and examination is mandatory for patients undergoing IV anesthesia for surgery. This appointment is to review your medical history, medications, allergies and to be sure you are healthy to undergo sedation. Please have nothing to eat or drink 6 hours prior to any surgery involving sedation. You will also need an adult to drive you home and be responsible for your care.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, asthma, artificial heart valves and artificial joints) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy).
X-Rays:
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at the time of consultation or surgery.
